Parent “Homeroom Request” Forms will be available April lst . The form consists of three information gathering questions: (1) List any academic information about your chiid that you would like considered when making a placement; (2) List any social, emotional, behavioral, information about your child that

you would like considered when making a placement; and (3) include any other pertinent information that you feel should be considered when making a placement for your child. This information will not only assist with placements, but also will provide teachers with pertinent information about students. 0n the last line, parents will have the option to name a preferred teacher. Parents can fill out the form

even if they do not choose to name a teacher. Parents can request a form from the starting April 1”. Please return all forms directly to the office by May 17 so that lists can be worked on prior to the end of the school year.
